After carrying out a Full Electronic Confirmation, you may be presented with an Action Required alert against one or more of the four verification sections.
To understand why the Action Required result has been returned, you should download the report and review the information provided. The report contains the full details of the result, helping you determine the appropriate next steps.

Why did my client not automatically pass the Address and DOB Verification check?
To pass this section electronically, the client must have at least one Credit Account Information Sharing (CAIS) match, plus either a match on the Electoral Register or a second CAIS match. In addition, there must be no indication that the client is deceased or no longer resides at the address.
Why did my client not automatically pass the Mortality Check?
The Mortality Check compares the client's first name, last name, and current address against the UK Deceased Persons Database to determine whether there is any indication that the individual has been recorded as deceased.
A failed or Action Required result does not necessarily mean that your client is deceased. It simply means the system has been unable to confidently rule out a potential match or could not verify the result automatically.
This can occur for a number of reasons, including:
- The client shares the same name as a deceased individual.
- The client's address information does not match the records held.
- There is insufficient information available to confidently verify the result.
